Students new to Wake County:

**All New Students Must Register At Their Base School Prior to Choosing A Year-Round Option Or Transfer **

Newcomers to Wake County must provide the following documents in order to complete the student's registration. Your student cannot be enrolled without the following and records cannot be requested until after enrollment. Please call the school to make an appointment to register your child. While not required, this can save you a lot of time.

  • Legal Proof of residence in the form of a current electric, city gas or water bill, signed lease agreement or closing statement in the name of the parent(s) or court-appointed guardian with a closing date within 45 days of enrollment (telephone, cable television bills and driver's licenses do not qualify). *
  • Parent/Guardian Photo ID.
  • A certified copy of the student's birth certificate.
  • Current Immunization records.**
  • A completed Discipline Status Enrollment form is required and must be notarized.
  • Copy of the child's latest report card or withdrawal grades from Previous School, if child is registering after school year has begun. A copy of the previous year's achievement test results is also helpful.
  • Copy of any custody and/or court order papers.
  • AG, IEP or Special Programs Information. (IEP is Mandatory if served in Special Programs)

To choose LRMS as your year-round option, the application process begins online February 9, 2009 through February 28, 2009. Please see below for more information on the appliction process.

 

 

 

Students applying to Lufkin Road Middle as their year-round calendar option:

If you plan to apply to Lufkin Road Middle School as your year-round calendar option, please complete the online application between February 9th - February 28th for next year enrollment in LRMS. This application can only be complete online. After you complete the online application, you will receive a formal offer from the Growth and Planning Office for a seat in LRMS. There is a designated period of time in which to accept or decline the seat. You must return the form indicating a decline of the assignment if that decision is made after the WCPSS offer. If the offered seat is not declined through submission of the form, LRMS assignment becomes the student's new school assignment for the full school year. If you receive an offer for LRMS and choose not to attend, you will be reassigned to your original base school assignment and will not be offered a second magnet/calendar-option school.

If your application is approved please bring the following when registering your child:

  • Approved Application Letter from WCPSS Growth and Planning.
  • Proof of residence in the form of a current electric, city gas or water bill, signed lease agreement or closing statement in the name of the parent(s) or court-appointed guardian with a closing date within 45 days of enrollment (telephone, cable television bills and driver's licenses do not qualify).*
  • Parent/Guardian Photo ID.
  • Copy of the child's latest report card or withdrawal form with withdrawal grades.

 

 

Students requesting a tranfer to Lufkin Road Middle School:

Students wishing to request a transfer to LRMS may do so by completing and sending the (2009-2010 School Year) Transfer Request Form to the Office of Growth and Planning . These forms are available in all schools beginning May 15, 2009 - June 1, 2009, at your base school's Student Services Department, the Office of Growth and Planning or from the WCPSS Website . This form cannot be sent online. Parent(s) or court-appointed custodian will receive written notification regarding approval or denial of transfer requests within seven to ten working days following receipt in the Office of Growth and Planning. If your transfer request is approved please bring the following when registering your child:

  • Approved Transfer Request Letter from WCPSS Growth and Planning.

Students who have moved and are enrolled at another WCPSS school:

Contact your current school to withdraw your student. Once withdrawn, please provide the following when registering your student at Lufkin Road Middle:

  • Proof of residence in the form of a current electric, city gas or water bill, signed lease agreement or closing statement in the name of the parent(s) or court-appointed guardian with a closing date within 45 days of enrollment (telephone, cable television bills and driver's licenses do not qualify).*
  • Parent/Guardian Photo ID.
  • Copy of the child's latest report card or withdrawal form with withdrawal grades.

 

*If student's family is living with relatives/friends, a Notarized Affidavit of Residence Form is required, along with the residence provider's Legal Proof of Residence.

 

** NC law requires a booster shot of Tdap for all students entering the 6th grade on or after August 1, 2008, if five years or more have passed since the last dose of tetanus/diphtheria toxoid. Free Clinics

Immunization Information

A booster dose of Tdap vaccine is required for all children before entering 6th grade that are enrolled in public school.  By North Carolina law, students who have not received the Tdap booster by the specified date are not allowed to attend school.  For students in year-round school those dates are as follows: Track 1- August 17, 2009; Track 2- September 8, 2009; Track 3- September 8, 2009; Track 4- September 4, 2009.  Please take the certificate of immunization to the current elementary school prior to June 30, 2009so they can enter the data in NCWise and record the date on your child's health card in their permanent cumulative folder. If unable to turn in the immunization certificate to your elementary school prior to June 30th, please bring the immunization certificate on July 6th.
