New Student Registration Information

Newcomers and those who have moved to a new school attendance area in Wake County must must provide the following documents in order to complete the student's registration. Your student cannot be enrolled without the following and records cannot be requested until after enrollment. Please call the school to make an appointment to register your child. While not required, this can save you a lot of time.

1. I.E.P (MANDATORY if served in Special Programs)
2. Legal Proof of Residence- Must be current utility bill, water or public gas, House lease/contract with current closing date, (within 45 days of enrollment) and signatures-No Exceptions. *
3. Most current report card.
4. Withdrawal form from previous school, if child is registering after school year has begun.
5. Original Birth Certificate
6. Immunization Certificate- Current shot records.
7. Parent/Guardian Photo ID
8. Copy of any custody and/or court order papers.**

5. Home Language Survey
6. Photographic/videotaping permission
7. Computer rules
8. Bus Route info.
9. Discipline Status Enrollment Form- (MUST BE NOTARIZED)***
10. Free or Reduce Lunch Application- if applicable and requested
11. Verification of child custody

*If student's family is living with relatives/friends, a Notarized statement from the parent stating where parent/child lives and a Notarized statement from residence provider, along with their Legal Proof of Residence, will be required.

**If student is living with relatives/friends, (other than legal parent(s) listed on Birth Certificate), the guardian must have court appointed papers. This includes notarized statements, social service papers, etc.

Welcome to Lufkin Road Middle School. We are pleased that you have chosen Lufkin Road for your child's school. If you child is a returning Lufkin Road student, the following information will help you complete the 2008-2009 regisitration forms that your child will be bringing home along with a packet of information in March. If you are new to Lufkin Road or are a transfer students please follow the above New Student Registration procedures prior to completing the grade level registration forms.

If you would like to request a track change, forms will be available in the Guidance Office.

High School Course selection Worksheet 

The Wake County Public School System's Student/Parent Access Network ( SPAN ) enables current 8 th Grade students to make course selections for next school year online. Students will select courses at both their base school and at any schools applied for (if applicable). Before you begin selecting courses, review your school's registration material for information on course selection. You should also think about the courses you are interested in taking next year and beyond. The following are copies of the area schools course selection forms to help in the registration process.
